Not least of the problems is the need to learn where all the weaknesses are. A new system will have new ones just waiting to be exploited. Support has to be trained up to cope and staff taught how to use it.
At least with an old system the loopholes and weaknesses are understood and in many respects this knowledge allows the system to work and be more robust. Too much reliance on a new system will always create a bigger disaster when it fails and that is because what causes that failure is new for everybody.
This is one of the biggest reasons why people are reluctant to change from what they know. They know how to cope with it when it breaks!
